The DGUV accounting form is an important document for companies and organisations that participate in measures to promote occupational health and safety. It is used for the proper accounting of training courses, training programmes or measures that are funded by the German Social Accident Insurance (DGUV). The use of this form enables companies to apply for the corresponding subsidies or reimbursements for the training courses and measures carried out.

Billing form

This billing form for first aid courses in accordance with DGUV guidelines serves as a means of payment for those participants in first aid training courses who are employed by companies, educational institutions and other organisations & have accident insurance through their employer. The German Social Accident Insurance (DGUV) requires that first aiders receive regular training in order to be able to provide qualified first aid in emergencies. This form is used to settle the costs incurred for these training courses with the accident insurance providers. The First Aid School Berlin takes care of this procedure and the participants in the first aid course bring the completed form with them on the day of the course.

Contents of the form

The form records all essential information about the training and the participants.

In principle, the entire green section must be completed by the member company. 

The yellow part is completed by the First Aid School Berlin. 

1. Information about the trainingDate and location of the first aid training course as well as the name and address of the training provider and the course number. 

2. List of participantsList of employees attending the course, valid ONLY with the signature of the PARTICIPANT to confirm attendance on the day of the course. If there are more than 10 participants, a second form must be completed. 

3.  Signature & stamp of the company: Only with contact data, membership number, stamp and signature of the company, Erste Hilfe Schule Berlin can submit the form for billing. 

4 BGW and BGN have special accounting forms that must be applied for separately.

1. Purpose of the form:

- The settlement form is used to claim the subsidies offered by the DGUV for training and further training measures in the area of safety and health at work.

- It enables the documentation of all relevant information on the measures carried out.

2. Who can use the form?:

- All DGUV member companies that participate in recognised training programmes or prevention measures.

- This includes companies from various sectors that would like to offer their employees relevant further training in the area of occupational safety.

3. Contents of the form:

The DGUV accounting form typically contains the following information:

• Company dataName, address, contact details of the company.

• Course and measure detailsTitle of the measure, date, duration, number of participating employees.

• Cost overviewBreakdown of the costs of the measure, any amounts already paid and the amount of reimbursement requested.

• Signature and confirmationConfirmation of the accuracy of the information by a responsible person in the company.

4. Submission of the form:

- The completed claim form should generally be sent to the relevant accident insurance fund or the relevant DGUV contact person.

- Please note the deadlines for submission to ensure that your enquiry can be processed on time.

5. Important notes:

- Make sure that all information on the form is complete and correct to avoid delays in processing.

- It is advisable to keep copies of all relevant documents (e.g. certificates of attendance, invoices) in order to be able to present them in the event of a subsequent claim.
